Responsibilities
- Coordinate patient care plans and weekend health screenings with multidisciplinary teams
- Provide direct patient education on chronic disease management and preventive care
- Lead community wellness workshops during weekend hours at local centers
- Maintain accurate electronic health records and documentation systems
- Collaborate with weekday staff to ensure seamless continuity of care
- Monitor and report on patient outcomes to improve service quality
- Participate in weekend on-call rotations for urgent patient inquiries
Qualifications
- Associate's degree in Nursing or Health Science (Bachelor's preferred)
- 2+ years clinical experience in a healthcare setting
- Valid North Carolina healthcare professional license
- Strong weekend availability with flexibility to work occasional holidays
- Excellent communication skills with diverse patient populations
- Proficiency in EHR systems (Epic experience a plus)
- CPR/BLS certification
- Ability to work independently while collaborating effectively with teams
